Kylie Jane Phillips, Jennie Apirana & Moana-o-Hinerangi comprise Te Ara Haumanu – the name given to the whānau ora kaupapa (Navigators) in RCPHO.
Te Ara Haumanu refers to the whānau wellness restoration journey as well as to the group of 3 – who together will open up whānau ora possibilities for the 4,785 members of whānau members currently self-identified and enrolled in practices.
Whānau ora – whānau wellness, is centred around seven pou or pillars – including whānau becoming self-managing and empowered leaders, whānau leading healthy lifestyles, and whānau confidently participating in Te Ao Māori – Māori society.
Referrals can come either through general practices or directly from whānau who are enrolled with an RCPHO practice, or are prepared to be. A referral process is being set up on the RCPHO website.
