The provision of Practice Support Navigators is a funded innovative response by the Rural Canterbury PHO to support General Practices/ Medical Centres in the excellent work they do with patients.

Practice Support Navigators seek to support General Practices in their work with all patients by enabling equity of access to services and empowering patients and whanau to have improved health outcomes.

Practice Support Navigators work in partnership with Whānau Ora Navigators – Te Ara Haumanu to achieve the best outcomes for Maori Whanau.
