Primary Care Mental Health Liaison (0-3% Mental Health)

 

Purpose:

To assist rural General Practice to appropriately identify, treat, refer and case manage mental illness in their practice population to achieve better quality of life for patients.

Key Components:

The Nurse Specialist will

  • act as a resource person
  • act as a liaison person between the primary care teams and specialist mental health services
  • provide education and treatment advice to rural GP teams.

Exclusion criteria:

This role overlaps with several PHOs.
