Medicines Use Review
Purpose:
To optimize health outcomes for patients by assisting those who are having difficulty use, and/or understanding, and compliance with medication.
Key Components:
- Identification of and addressing any problems with effectiveness and utilization of current medications by individual patients
- Support patients and their whanau/families to better understand and manage their medicines and medical conditions
- Assist patients and their whanau/families to self-manage their medicines for chronic medical conditions
Inclusion criteria:
The Person must have:
- one or more chronic disease states
- 2 or more co-morbidities
- 1 or more of the following conditions:
- takes 4 or more medications and/or 12+ doses a day
- at increased risk of medicine-related problems including:
- significant changes in meds regime in last 3 months
- taking (or about to start) meds with a high risk of adverse effects
- symptoms of medication adverse effects
- sub-optimal response to pharmacotherapy
- non-adherence or unable to manage their meds.
- literacy, dexterity, vision, cognitive or language difficulties that impair ability to manage their meds.
- multiple prescribers
- recent admission to hospital (within 4 weeks) with medication changes
