Chronic Disease Management – Packages of Care

2 programmes - 1 for Maori, and 1 for non-Maori

Purpose:

To support people newly diagnosed or newly discharged with chronic conditions.Key Components:Care Packages are tailored to the individual person's needs

How to access :

Complete the Chronic Disease Management - Packages of Care registration form and send to the PHO office.
Complete the Chronic Disease Management - Packages of Care referral form as necessary and send to the appropriate service provider.
