Child Health Liaison Nurse

Aims:

• To assist RCPHO in facilitating health gains for children and their families.
• To reduce health disparities within the RCPHO enrolled population.
• To make the best use of current resources by maintaining links with, but not duplicating, existing health services.
• To identify children and/or families with specific health issues.
• To provide information to GPTeams re services available.
• To provide effective and timely liaison between GP Teams and community based agencies
• to fortify links with secondary care services, improving communication when a child is referred, admitted or discharged

Exclusion criteria:

People 15 years of age and over.
