Care Plus

Purpose:

This programme has a proactive outcomes focus to assist those with 2 or more chronic diseases to have low cost access and flexible packages of care. Allows four (4) extended visits using a CarePlan that is developed in consultation with the patient.

Key Components:

The person must meet at least 1 criteria of having

• been assessed as being able to benefit from PHC intensive clinical management
• 2 or more chronic conditions with significant morbidity and cost to the health system
• a terminal illness
• had 2 acute medical or mental health hospital admissions in past 12 months (not surgical admissions)
• received 6 first level or casual GP consultations and/or emergency department attendances in the last 12 months
• active review while waiting for elective health services
