Banks Peninsula - Green Prescription

A pilot programme to trial one on one clinic and home visit support for Green Prescription in Banks Peninsula in place of telephone support only; telephone support still to be used.

Key Components:

GRx consultant will enhance the existing four month GRx phone support service by:

1. Home based GRx consultation (max two per patient) leading onto phone support
2. General practice based GRx consultations (max two per patient) leading onto phone support
3. Phone support service only
