Lactation Consultant Referral Form September 2011 

Please supply as much relevant detail as possible to assist with triaging.  For appropriate reasons for referral, the baby's birth weight and current weight should be provided, as these are significant pieces of information (eg insufficient milk supply, slow weight gain / failure to thrive).

Support available for pregnant or breastfeeding mums

Having a baby? Already breastfeeding a baby of any age? Do you feel you would like support with your breastfeeding? Then contact with a volunteer breastfeeding peer supporter will be a positive experience for you.  We have trained supporters available for phone, text, email and face-to-face support.  This free service is provided by mothers who have breastfed, are passionate about breastfeeding and are trained to assist you with the ups and downs of breastfeeding - no question is too small.  Breastfeeding is great for you and your baby, so get to know a Mother4Mother supporter near you.  If you are interested in accessing this service please contact Rachel Palmer at Rural Canterbury PHO on 0800 800 743 ext 212, text 027 588 1007 or email:  rachel_palmer@rcpho.org.nz
