Acute Demand Management

Purpose:

To provide acute care to people who present in your location, and who are enrolled in any general practice in the RCPHO, CPHO, H-KPHO, CCPHO, Partnership PHO.

Key Components:

Acute Packages of Care

These services are generally delivered at the GP practice through extended consultations or practice based observation. Home visits may qualify through an extended consultation or practice based staff delivering care at the patient's home e.g. IV antibiotics.

Acute Nursing Services

Generally these nursing services are delivered in the patient's home by the relevant acute nursing provider. Acute nursing may also be delivered at the practice by the acute nursing provider e.g. when the practice requests nursing back up for an acute patient.For Waimakariri patients, an Observation Service is available at 24 hour Surgery, cnr Bealey Ave and Colombo Street. Doctors may refer patients to this observation service. Patients requiring acute nursing after 11pm will need to be transferred to 24 hrs or to hospital.

Exclusion criteria:

1. ACC

2. Non-enrolled patients in one of the above PHOs

3. Out of region (CDHB) patients

4. Overseas residents
